I have both asthma and arthritis. The arthritis is quite painful, but when I try to buy ibuprofen from a pharmacist, they refuse because I have asthma. I have taken ibuprofen many times with no problems. Should I avoid it?

Dr Fred Kavalier answers your health question:

About 20 per cent of adults with asthma have what is called aspirin induced asthma. They develop potentially severe asthma symptoms if they take aspirin or non-steroidal anti-inflammatories such as ibuprofen. Asthmatics who are sensitive to aspirin and NSAIDs often have nasal polyps, an allergic skin rash called urticaria, and chronic nasal allergies. If you have ever had an allergic or asthmatic reaction to these drugs, you should never take them again. But if you are over 40 and have taken ibuprofen with no problems, the risk in taking it again is very small. As a rule, asthmatics should use paracetamol. If you really need ibuprofen, be aware of any effect it has on your asthma. If you ever feel it makes it worse, do not take it. All children, asthmatic or not, should avoid aspirin.

Medications To Avoid With Asthma

Various types of medications are used to treat asthma, including bronchodilators and steroids. But just as there are drugs you can take to treat asthma symptoms, there are also certain medications to avoid with asthma.

It is essential to understand that everyone with asthma is different. People have different drug sensitivities. A medication that affects one person adversely may not affect someone else the same way. In general, there are specific classifications of medications that people with asthma should avoid or talk to their doctor about before taking.

If You Suspect You Might Have Asthma Definitely Head To The Doctor

Your doctor will probably give you a physical exam first to examine the general state of your health. After that, theyll likely put you through some lung function tests, such as a spirometry, which checks how much air you can exhale after taking a deep breath as well as how fast you can expel air, according to the Mayo Clinic. Or they may do a peak flow test, which measures how hard you can breathe out. If you cant exhale enough air or breathe out quickly, it may be a sign your lungs arent working well, which could point to asthma, Dr. Benzaquen says.

There are other exams they can use, too, like exposing you to methacholine, a known asthma trigger, to see if your airways narrow, or allergy testing, since allergies and asthma are so often connected.

If you are diagnosed with asthma, itll be within one of four categories, according to the Mayo Clinic. Mild intermittent asthma means you have minimal asthma symptoms for up to two days a week and up to two nights a month, while mild persistent asthma means youre experiencing symptoms more frequently than twice a week, but not more than once on any given day. Moderate persistent asthma ups the ante: Youre dealing with symptoms once a day and more than one night a week. Finally, severe persistent asthma involves constant symptoms most days and frequently at night too.

Will Aspirin And Other Pain Relievers Make My Asthma Worse

Aspirin and other drugs called nonsteroidal anti-inflammatory drugs may be harmful for people who have asthma. Ibuprofen and naproxen are a few examples of NSAIDs. If you are allergic to aspirin, ask your doctor or pharmacist to make sure any new medicine you might take is not related to aspirin.

People who have asthma can usually take acetaminophen safely. This medicine is typically used to relieve fever and pain. Very rarely, even acetaminophen may make asthma worse, which has prompted further studies to explore the link between acetaminophen and asthma. If acetaminophen makes your asthma worse, tell your doctor. He or she can help you find another type of pain reliever.

Reliever Medication For Asthma

Reliever medication is used for an asthma flare-up or emergency. It opens the airways quickly and is taken as needed for immediate relief from asthma symptoms.

Reliever medication is sometimes overused. Using too much, or too often, could lead to side effects, including tremors and a rapid heart rate.

These side effects are not likely to cause harm. However, frequent use of reliever medication may be a sign that your asthma is not being well controlled. If you need to use a reliever more than twice a week for asthma symptoms , see your doctor to review your asthma and medication.

How Do Steroids Help Asthma

The steroids used to treat asthma are known as corticosteroids. Corticosteroids are copies of hormones your body produces naturally.

Steroids help asthma by calming inflamed airways and stopping inflammation. This helps ease asthma symptoms such as breathlessness and coughing. It will also help prevent your lungs reacting to triggers.

Youre more likely to avoid high doses of steroids if you take your preventer inhaler every day as prescribed, says Dr Andy Whittamore, Asthma UKs in-house GP.

Ive been told since Ive been diagnosed with asthma that I should now avoid anything with ibuprofen in, well the other day my GP prescribed ibuprofen gel for pain relief as I have carpal tunnel, she told me the gel is fine to use as it doesnt enter the blood stream like the tablet form or injection, I used it twice once in the middle of the night and again once in the morning, both times Ive been breathless and needed my blue inhaler. Is this a coincidence or am I thinking too much about it

I used gel and I am fine with it. Tablet is the problem for me. You might had a nightmare that set your off

I can’t use any gels or creams either as they trigger my asthma

Ibuprofen can definitely cause breathing issues in many asthmatics, personally for me it doesnt affect me at all, tablet or otherwise.

For the gel to work it has to pass through the skin and into the deeper tissues, muscle, joints and tendons etc, once this deep it would be impossible for some of it to not get into the blood stream but it would be in a very small dose.

I think it is unlikely the gel is causing your problems but not impossible.

Thats what the dr told me, but its been a strange coincidence, Im back up the drs today to see if they can give me something different, I find it quite scary when my breathing plays up, Im newly diagnosed asthmatic after a bad bout of pneumonia last year so its all still so new to me, before the pneumonia I was perfectly fine with ibuprofen.

How To Take Etoricoxib

  • Before you start taking etoricoxib, read the manufacturers printed information leaflet from inside the pack. It will give you more information about the tablets, and it will also provide you with a full list of the side-effects which you could experience from taking them.
  • Take etoricoxib once each day, exactly as your doctor tells you to. There are four strengths of tablet available 30 mg, 60 mg, 90 mg and 120 mg. You will be prescribed the strength of tablet that suits your condition. People with osteoarthritis are usually prescribed 30 mg once-daily, although the dose can be increased to 60 mg if needed. People with rheumatoid arthritis or ankylosing spondylitis are prescribed 60 mg once-daily, although the dose can be increased to 90 mg if it becomes necessary. If you are taking etoricoxib for gout, you will be prescribed a short course of 120 mg strength tablets to take once-daily for up to eight days.
  • Swallow the tablet with a drink of water. You can take the tablet either with or without food, although the tablets may work more quickly if they are taken without food.
  • Try to take your doses at the same time of day each day as this will help you to remember to take them.
  • If you forget to take the tablet at your usual time, take it as soon as you remember. If you do not remember until the following day, leave out the forgotten dose from the previous day and take the dose that is due as normal. Do not take two doses at the same time to make up for a missed dose.

Asthmatics With Nasal Polyps Should Avoid Nsaids Others Use With Caution

How to correctly use an asthma inhaler

NSAID-induced bronchospasm should be suspected in any patient whose asthma control worsens on initiation of a NSAID. Patients with a history of asthma should be warned of this reaction and to seek medical help if symptoms worsen on initiation of a NSAID.

NSAIDs should be used with caution in the presence of asthma and avoided in asthmatics with nasal polyps. As it is difficult to identify ‘at risk’asthmatics, it would seem prudent to prescribe paracetamol instead of aspirin unless there are any specific contra-indications.

Patients should be reminded to read labels of over-the-counter medicines as some, such as cough/cold preparations, may contain aspirin. Ibuprofen, diclofenac and other NSAIDs are also available over-the-counter.
