• Mail, fax, or drop off your school, daycare, and/or camp forms to our clinic
• Before submitting to our clinic, please ensure you’ve reviewed and thoroughly filled out your form with as much information as possible. This will help ensure our staff is able to swiftly complete the healthcare provider’s information on your form
• Patients/parent may request their Asthma Action Plan or Emergency Action Plan by calling our clinic at 952-546-6866
• Please allow 2 weeks for forms to be completed
• Forms will either be returned to the familys home address on file via mail OR the family may pick up forms in the clinic
• We do not fax or email form directly to your institution/organization. Each family/patient is responsible for submitting forms to their specific school, daycare, and/or camp

Southdale Allergy and Asthma Clinic is one of the oldest allergy offices in the Twin Cities. Dr. Albert Stoesser, an associate professor at the University of Minnesota, opened the clinic in 1948. Our office takes exceptional care of our patients, and carefully attends to patients individual needs. We keep our surroundings comfortable.

Plymouth Office Closing June 30

Dear Patients,

It is with a bit of sadness that we need to inform you that our WestHealth/Plymouth location will close permanently on June 30, 2022.

We will continue your allergy and asthma care at our Minneapolis location or at the new St. Louis Park ENT Specialty Care location.

If you are on allergy immunotherapy both locations will continue to provide these services to you. If you would prefer getting your shots at your primary care clinic and they agree to provide this service, please call us to send your serum to them.

It has been our pleasure to serve you over the past 25 years at WestHealth/Plymouth. We look forward to continuing your care in Minneapolis or St. Louis Park. Please call for appointments.

At Canton Asthma & Allergy our doctors provide unsurpassed care for children and adults affected by allergies or asthma. Our physicians provide evaluation, testing, diagnosis and treatment for a host of breathing and allergic conditions.

• Please CALL before coming to the office:
• If you are feeling sick.
• If you have a fever.
• If you have aches, cough, sore throat, and/or shortness of breath.

Biologic and Allergen Injection Patients

Due to the COVID-19 pandemic, we have implemented the following changes for your safety:

• After receiving either a biologic or allergen injection, patients have the option to wait 20 minutes in their vehicle or in our office. Exceptions include a 60 minute in-Office wait for the first dose of a biologic or a physician chosen wait time for someone who has had a previous reaction to an injection. If you choose to wait in your car, you must have your Epinephrine injector with you, the same as a patient who waits in the office.
• Prior to leaving the office to wait 20 minutes in your car you will be asked to complete a waiver, and to provide a cell phone number where we can reach you.
• You must return to the office or call 911 if you experience symptoms of anaphylaxis.
