Cough And Airway Remodeling: A Possible Link
We aimed to investigate whether repeated cough itself induces airway remodeling and heightened CRS by using conscious guinea pigs in vivo . Cough was induced by repeated exposure to 0.5 M citric acid thrice weekly for 3 weeks , which is unlikely to induce tachyphylaxis. We set two control groups: one given saline , and the other given CA with pretreatment with an antitussive dextromethorphan according to the same protocol as CA group . Average cough number for 10 CA exposures was evaluated as âcough indexâ in the CA group, which was divided into frequent cough and infrequent cough subgroups for BAL and pathology analysis of day 25. The number of coughs significantly increased in the CA group from day 13 onwards, but not in the SA or CA + DEX groups. Among various histologic changes, ASM area of small airways was significantly greater in the CA-F subgroup than in the other three groups , and correlated with cough index in CA group . BAL cell profiles were similar among the four groups. We concluded that repeated cough induces ASM remodeling in the absence of inflammation. It was associated with heightened CRS, leading to a positive feedback mechanism for cough persistence .

Causes And Complications Of Cough Variant Asthma
Cough variant asthma is pretty specific and is easily noticeable in young children. Much like classic asthma, the true causes of cough variant asthma are currently unknown however, it does seem to appear during upper respiratory infections, following contact with allergens, and among those who have started taking beta blockers .
The cough will essentially have the same characteristics as traditional asthma, in that the breathing tubes will react to irritation by spasming, or squeezing into a smaller size. The cough will also be nonproductive, producing no phlegm or mucus.
Kids with cough variant asthma may have a cough caused by the common cold, acute bronchitis , or another upper respiratory infection that doesnt seem to go away. This cough will continue long after the initial virus has disappearedfor at least four weeks in children and eight weeks in adults. Cough medicine will also not work on this cough. And if untreated, cough variant asthma can come with some complications including sleep disruption, hoarseness and sore throat, and headaches.
Risk factors that can increase your chances of getting cough variant asthma include:
- Having allergies
- Smoking or exposure to secondhand smoke
- Exposure to environmental irritants
- Having asthma or a family history of asthma

Coughing expels mucus, microbes, and foreign particles from the respiratory tract, protecting the lungs from infection and inflammation.
The cough begins with an initial gasp that draws air deep into the lungs. Next, the glottis snaps shut, putting a lid over the trachea, or windpipe. The third step is the forceful contraction of the muscles of the chest cage, abdomen, and diaphragm . In normal breathing, these muscles push air gently from the lungs up through the nose and mouth. But when the glottis is closed, the air cant move out, so tremendous pressure builds up in the air passages. Finally, the glottis swings open and the air rushes out. And it is quite a rush in a vigorous cough, the air travels out at nearly the speed of sound, creating the barking or whooping noise that we call a cough.

How Is Cough Variant Asthma Treated
Over-the-counter medicine doesnt target airway issues, and most people with CVA dont get any relief from it. Therefore its crucial to get diagnosed and treated by a healthcare professional.
Treatment for CVA usually involves a similar approach to asthma and consist of a combination of drugs, including:
- Allergy medication.
- Corticosteroids or anti-inflammatory drugs that ease and prevent swelling.
- A beta antagonist to prevent future asthma attacks.
In addition to medication, your doctor may advise you to avoid whatever triggers an attack, like smoke, pollen, or pet dander.
Clinical And Physiological Features And Treatment Of Cva
CVA or asthma has consistently been one of the most common cause of chronic cough worldwide, together with GERD and upper airway cough syndrome. In Japan, CVA is the leading cause followed by GERD and sinobronchial syndrome , while one study from Poland showed a marked predominance of GERD followed by upper airway cough syndrome, probably indicating a regional or racial difference .
The prevalence of atopy in CVA, as defined by the presence of at least one positive serum specific IgE or skin test response to common aeroallergens, ranges from 40% to 80% , similarly to classic asthma. Interestingly, in a follow-up study of patients regularly visiting our clinic of Kyoto University for more than 2 years, repeated seasonal exacerbations in a specific season were more common in CVA than in classic asthma . Patients with CVA had seasonal exacerbations more frequently in autumn season than those with classic asthma . The latter phenomenon in CVA was associated with sensitization to housedust-mite .

Symptoms Of Cough Variant Asthma
- The only characteristic feature of a case of CVA is an unexplained chronic cough that doesnât seem to be rooted in any other condition or cause.
- This cough is usually dry, which means it is not accompanied by phlegm or mucus expulsion and lasts at least six to eight weeks.
- Although CVA does not present any of the other symptoms typically associated with asthma, such as fluid in the lungs, chest tightness, shortness of breath, and wheezing, it affects the airways in the same way as its more traditional counterpart does. To expand further, the bronchial tubes tend to become hyper-responsive and tighten up, making breathing increasingly difficult. Thus, itâs essential to manage CVA properly to avoid grave complications.
How Common Is Cough In People With Asthma
Asthma is the second most common cause of chronic cough.4 It is responsible for chronic cough in 24% to 29% of adult non-smokers.4
Cough is the most commonly reported asthma symptom.3 In one study, 88% of people who went to the emergency room for an asthma flare-up reported cough as one of their symptoms.5 It may last for up to three weeks after an asthma attack.3
For about 57% of people with asthma, it is the only symptom they have.3 This type of asthma is called cough-variant asthma.3

HOUSTON Forced exhaled nitric oxide, or FeNO, may be useful for diagnosing cough variant asthma in patients with chronic cough who have normal spirometry, researchers reported at the American College of Allergy, Asthma and Immunology Annual Meeting.
We know that cough variant asthma is a type of asthma in which the main symptom is a dry, nonproductive chronic cough in the absence of wheezing or shortness of breath. The condition is frequently underdiagnosed since baseline spirometric volumes are usually normal,Talal M. Of the patients with chronic cough, 91% had elevated FeNO measurements with mean average values of 64.4 ppb vs. the optimal normal cutoff value of 25 ppb and 60% had abnormal ACT scores of 15.4, according to the results. In contrast, control patients had normal FeNO measurements with mean average values of 16 ppb and normal ACT scores.

What Are The Symptoms Of A Cold
When youve got a mild cold, the only symptoms might be a runny nose, mild sore throat and cough, and general fatigue. If your cold is more serious, you may also have body aches, and all over pains, a fever, trouble sleeping, and your cough and sore throat may be worse.

Also, having a low amount of red blood cells can cause shortness of breath that is unresponsive to asthma therapies and would require further evaluation to determine the cause. Certain blood tests can also look for allergiesor infections that might be the cause of your symptoms.
