  • Asthma is usually considered a periodic disease. This means symptoms only occur sometimes. COPD is usually considered a persistent disease. This means that symptoms may appear to some degree all the time.

Clinical Trials Involving The Anticholinergic Drug From Boehringer Ingelheim

The clinical development programme for spiriva included more than 25,000 patients. Phase III trials of spiriva included two one-year placebo-controlled trials, two one-year active-controlled trials and two six-month placebo controlled trials.

The studies recruited 2,663 patients, aged 39 to 87 years. The most common adverse reaction in the trials was dry mouth.

Two additional trials, including a six-month randomised placebo controlled study in 1,829 patients, were conducted. The second study was a four-year trial, called Uplift, carried out in 5,992 patients.

The trial showed that spiriva improved lung function and quality of life for patients. The patient population and adverse reaction were similar to that of the one-year and six-month trials.

Based on the results of the Uplift study, the FDA approved spiriva for reduction of exacerbations in patients with COPD.

In March 2011, results from a new one-year study, POET-COPD, were announced. The study was conducted to compare the affects of Salmeterol with spiriva in reducing COPD exacerbations . It recruited 7,376 patients with moderate to severe COPD. Results from the study indicated that spiriva was superior in reducing COPD exacerbations, compared to Salmeterol.

Side Effects In Children

Spiriva Respimat is used to improve lung function in children with asthma who are ages 6 years and older. This drug is used as a maintenance treatment to control asthma symptoms over time.

In clinical studies, there were no differences in the common side effects seen in children and teenagers using Spiriva Respimat compared to adults using the drug.

Spiriva HandiHaler isnt approved for use in children.

A pulled hamstring that was once a week recovery takes about a month. And that snap, crackle, pop sound is your body and not rice krispies. This leads me to a disease that really begins to hit hard once you are about 40 and can be really debilitating as you get into your 60s. It is called COPD or chronic obstructive pulmonary disease. This is not a single disease but a combination of diseases all rolled up into one. If you suffer from this, there is a good chance you might be using Spiriva to manage it.

Before Taking This Medicine

You should not use Spiriva if you are allergic to tiotropium or ipratropium .

To make sure this medicine is safe for you, tell your doctor if you have ever had:

Tell your doctor if you are pregnant. It is not known whether tiotropium will harm an unborn baby. However, having uncontrolled asthma during pregnancy may cause premature birth, a low birth weight baby, or complications such as eclampsia . The benefit of treating asthma may outweigh any risks to the baby.

It may not be safe to breast-feed while using this medicine. Ask your doctor about any risk.

Spiriva Respimat is not approved for use by anyone younger than 6 years old.

Drug Forms And Administration

Spiriva comes in two forms that are taken as follows:

  • Spiriva HandiHaler. This form of Spiriva comes as powder-filled capsules that are taken using an inhaler. It should be taken as two inhalations , using just one powder capsule for both puffs, once each day.
  • Spiriva Respimat. This form of Spiriva comes as a liquid solution inside an inhaler. The strength of Spiriva Respimat that youre given depends on whether youre taking it for asthma or COPD. This drug should be taken as two inhalations once each day.

Incruse Ellipta comes as an inhaler that contains a powder. It should be taken as one inhalation once each day.

Side Effects And Risks

Spiriva and Incruse Ellipta both contain drugs that belong to a class of medications called anticholinergic agents. Therefore, these medications can cause very similar side effects. Below are examples of these side effects.

How To Use It

Spiriva Respimat is for inhalation use only.

The Spiriva Respimat inhaler releases a mist of solution slowly and gently, making it easy to inhale it into your lungs where it is needed. The mist containing the medication is released by pressing the dose release button.

How Should I Use This Medication

For adults and children 18 years of age and older, the recommended dosage of tiotropium is to inhale the contents of one 18 µg capsule once daily through the HandiHaler® inhalation device.

Many things can affect the dose of medication that a person needs, such as body weight, other medical conditions, and other medications. If your doctor has recommended a dose different from the ones listed here, do not change the way that you are taking the medication without consulting your doctor.

Take this medication at the same time each morning. The capsule shell should be discarded after the medication has been taken. Do not swallow the capsule shell.

Your doctor or another health care professional, such as your pharmacist, should teach you how to use the HandiHaler® inhalation device. If you are unsure of how to use the device, contact your doctor or pharmacist.

It is important to take this medication exactly as prescribed by your doctor. If you miss a dose, take it as soon as possible and continue with your regular schedule. If it is almost time for your next dose, skip the missed dose and continue with your regular dosing schedule. Do not take a double dose to make up for a missed one.If you are not sure what to do after missing a dose, contact your doctor or pharmacist for advice.

Store this medication at room temperature, protect it from light and moisture, and keep it out of the reach of children.

How Should I Use Spiriva

Use Spiriva exactly as prescribed by your doctor. The usual dose of this medicine is 2 inhalations through the mouth once daily. Follow all directions on your prescription label. Do not use this medicine in larger or smaller amounts or for longer than recommended.

Spiriva is not a rescue medicine. It will not work fast enough to treat a COPD flare-up. Your doctor may prescribe a fast-acting inhalation medicine to treat a bronchospasm attack. Tell your doctor if it seems like your medications don’t work as well.

Push the button on the side of the device to pierce the capsule and release the medicine into the inhalation chamber. You will need 2 inhalations to get the full dose from 1 capsule.

Do not take the capsule by mouth. Spiriva capsules are for use only in the HandiHaler device. Use only 1 capsule at a time. Do not use the HandiHaler device to take any other medicine.

To be sure this medicine is helping your condition, your lung function will need to be tested on a regular basis.

Large International Study Of Copd Drug Finds Two Types Of Inhalers Equally Safe And Effective

An international study led by a Johns Hopkins pulmonary expert finds that the drug tiotropium , can be delivered safely and effectively to people with chronic obstructive pulmonary disease in both “mist” and traditional “dry powder” inhalers.

The new Respimat inhaler, which delivers the drug in a mist form, is approved for use in Europe but not in the United States. The traditional inhaler, known as a HandiHaler, uses a dry powder form of the drug and is widely used in the U.S.

The study comparing the two drug-delivery systems was a randomized, double-blind, prospective trial conducted at 1,200 sites in 50 countries with more than 17,000 COPD patients. Results of the study are described in an article in the New England Journal of Medicine on Sept. 8, 2013, and are scheduled to be presented the same day at the European Respiratory Society meeting in Barcelona, Spain, by Robert Wise, M.D., professor of medicine at the Johns Hopkins University School of Medicine.

“This was a much-awaited study, because previous, smaller studies had raised the question of whether there was an increased risk of death with the newer tiotropium device, says Wise, who is the lead author of the study. “We needed a very large trial to answer the question.”

The researchers randomly assigned participants to one of three groups. Two different doses of the drug were studied among those who used the new Respimat inhaler.
