Severe Asthma Exacerbations Associated With Short Acting Beta Agonist Use
Increasing exposure to short-acting beta 2 agonist rescue or reliever medication appeared associated with greater risk for severe asthma exacerbations regardless of inhaled corticosteroid use, according to study results. Published in The Journal of Allergy and Clinical Immunology: In Practice, the study results suggest that rescue/reliever management using short-acting beta 2 agonist monotherapy should be avoided, according to the researchers.
Jennifer K. Quint, MD, PhD, FRCP, professor of respiratory epidemiology at Imperial College London National Heart and Lung Institute, and colleagues examined data from 1,033,564 patients in the SABa use IN Asthma series of observational studies including 10 North American and European datasets. Patients were aged 12 years and older with asthma and had been prescribed or possessed at least one SABA canister per year. Mean ages ranged across the datasets from 23.2 years to 72.2 years , and most patients were women .
The exception was the dataset based on Medicare findings, in which patients who were prescribed or who possessed one or two canisters per year saw higher incidence of severe exacerbations than patients with three or more canisters per year . Patients who had GINA scores of three to five and three or more prescriptions or possessions of SABA canisters per year experienced more severe exacerbations than patients with fewer canisters per year across all countries and datasets.

Asthma Increases Risk Of Hospitalization For Children With Covid
Researchers identified asthma as a risk factor for hospitalization among children who had COVID-19, but not for worse COVID-19 outcomes, according to a nested case-control study published in Pediatric Allergy and Immunology. The researchers also found that SARS-CoV-2 did not seem to be a strong trigger for pediatric asthma exacerbations, nor did asthma severity correlate with higher risk for COVID-19.

As the pandemic started, spread and eventually reached Allegheny County, in March 2020, there were many unanswered questions about how the virus affected children, and particularly those with underlying chronic conditions, study author Erick Forno, MD, MPH, ATSF, associate professor of pediatrics at University of Pittsburgh School of Medicine, told Healio.
A multidisciplinary group of physicians and nurse practitioners at University of Pittsburgh Medical Center Childrens Hospital of Pittsburgh and Childrens Community Pediatrics in Pittsburgh then launched the Pediatric COVID-19 Registry to guide conversations with patients and families so providers would not need to rely on anecdotal memory. The registry helped us understand how the pandemic affected children locally, and asthma was one of the diseases we focused on, said Forno, who also is director of the UPMC Childrens Hospital of Pittsburghs Pediatric Asthma Center.

Chemicals Found In Household Products May Increase Risk Of Asthma In Adults
Commonly-used household products should carry a warning that they increase the risk of asthma, according to a new evidence review. New research conducted by Smartline, a research project funded by the European Regional Development Fund, finds evidence that a group of chemicals found in a wide range of products in peoples homes increases the risk of asthma. Authors conclude that labelling should reflect this risk and warn people to ventilate their homes while using them. The research reviewed 12 studies into Volatile Organic Compounds , which are emitted as gases from certain solids or liquids. VOCs are emitted by a wide array of products, including some that are widely used as ingredients in household products. Paints, varnishes and wax, many cleaning, disinfecting, cosmetic, degreasing and hobby products may all contain ingredients that emit VOCs. Concentrations of many VOCs are consistently up to ten times higher indoors than outdoors.
One highly regarded study found a 15 per cent increased risk of asthma with exposure to Limonene, a naturally occurring aliphatic compound found in products like shampoos, detergents and air fresheners. Wood and kitchen paint also seem to increase symptoms.

Study Questions Hygiene Hypothesis Regarding Increase In Allergies
The theory that modern society is too clean, leading to defective immune systems in children, should be swept under the rug, according to a new study by researchers at UCL and the London School of Hygiene & Tropical Medicine. In medicine, the ‘hygiene hypothesis’ states that early childhood exposure to particular microorganisms protects against allergic diseases by contributing to the development of the immune system. However, there is a pervading view that Western 21st century society is too hygienic, which means toddlers and children are likely to be less exposed to germs in early life and so become less resistant to allergies.
In this paper, published in the Journal of Allergy and Clinical Immunology, researchers point to four significant reasons which, they say, disprove this theory and conclude we are not “too clean for our own good.” Lead author, Emeritus Professor of Medical Microbiology Graham Rook , said: “Exposure to microorganisms in early life is essential for the ‘education’ of the immune and metabolic systems. “Organisms that populate our guts, skin and airways also play an important role in maintaining our health right into old age: so throughout life we need exposure to these beneficial microorganisms, derived mostly from our mothers, other family members and the natural environment.
In a review of evidence, the researchers point to four factors.

Nearly 30% Of Peanut Allergies And 90% Of Egg Allergies Resolve By Age 6
About three out of 10 peanut allergies and nine of 10 egg allergies resolved by age 6 years, although infants with early-onset severe eczema or multiple allergies were less likely to outgrow them, according to a longitudinal study.
These findings may inform the use of disease-modifying treatments such as oral immunotherapy or proactive treatment of eczema in early life, the researchers wrote in the study, which was published in The Journal of Allergy and Clinical Immunology.

Prioritizing research of these and future interventions for infants less likely to naturally outgrow their allergy would yield the most benefit for health care resources and research funding, Rachel L. Peters, PhD, associate professor, epidemiologist and team lead within the population allergy research group at Murdoch Childrens Research Institute, Royal Childrens Hospital, in Parkville, Victoria, Australia, said in a press release.
Among the 471 children with raw egg allergy at age 1 year, allergies resolved in 89% of those with definite allergy by age 6 years. Additionally, compared with children whose egg allergy had resolved, persistent egg allergy was more likely among children with an SPT result of 4 mm or larger , peanut or sesame food sensitizations , early-onset severe eczema and baked egg allergy when aged 1 year.

Food Diversity In The First Year Of Life Leads To Fewer Allergies In The Second Year
Infants who ate a more diverse diet during their first year had lower risk for allergic diseases during their second year, according to a study published in Pediatric Allergy and Immunology.
Chunrong Zhong, PhD, of Tongji Medical College at Huazhong University of Science and Technology in Wuhan, China, and colleagues surveyed 2,251 women who gave birth to singleton live infants at 1, 3, 6, 12 and 24 months after birth.

The women were asked about breastfeeding, use of formula, complementary foods introduced and history of food allergies among their infants. The researchers then assessed food diversity by the number of food groups introduced to each infant. For the 6-month interview, the researchers categorized complementary foods into cereal, vegetable, fruit, meat, fish and egg groups. They found that 44% of the infants had been introduced to three to six of these groups. Cereals represented the most introduced food group, followed by fruits, egg yolk and vegetables.
In models adjusted for food allergy history of infants and other potential confounders, the researchers found significant inverse dose-response relationships with food diversity by 12 months of age and later allergic diseases , especially when those diseases involved the respiratory tract . Infants only exposed to one to five food groups by 12 months of age also had an increased odds of food allergy during 1 to 2 years of age .

Children Can Achieve Clinical Remission Of Peanut Allergy With Oral Immunotherapy
Oral immunotherapy with or without probiotic adjuvants conferred sustained unresponsiveness to peanut among allergic children, according to a randomized phase 2b study published in The Lancet Child & Adolescent Health. Although the probiotic adjuvant did not improve remission rates, it may improve the safety and tolerability of OIT, research found.
It is the first randomized controlled study of peanut oral immunotherapy to show high rates of sustained responsiveness in school-aged children and the first to show that a peanut allergy treatment can provide substantial improvement in quality of life compared to current standard of care of peanut avoidance, author Mimi L.K. Tang,MBBS, PhD, FRACP, FRCPA, FAAAAI, CEO of Prota Therapeutics and professor at Murdoch Childrens Research Institute in Melbourne, Australia, said in a press release.
The studys rollout
The 4-year study involved children aged 1 to 10 years with confirmed peanut allergies, with 79 participants assigned a probiotic and PRT120 peanut oral immunotherapy , 83 participants assigned a placebo probiotic and peanut OIT, and 39 participants assigned placebo probiotic and placebo peanut OIT at three hospitals in Australia. On day 1, participants received 2 x 1010 colony-forming units of Lactobacillus rhamnosus ATCC 53103 probiotic or placebo before receiving increasing doses of peanut protein or placebo every 20 minutes until they reached a dose of 12 mg, with a cumulative total of 24 mg.

Children With Asthma Not At Increased Risk Of Covid
In a new study published in Pediatrics, researchers concluded that children with asthma have similar risk for for SARS-CoV-2 infection compared with children without asthma.We found no evidence that children with asthma are at higher risk of COVID-19 or of developing severe illness from COVID-19, Matthew Kelly, MD, MPH, associate professor of pediatrics and global health, associate program director of the Pediatric Infectious Diseases Fellowship and associate director of physician-scientist development in the Office of Pediatric Education at Duke University School of Medicine in Durham, North Carolina, told Healio.
Kelly and colleagues conducted a retrospective cohort study that included 46,900 children aged 5 to 17 years in the Duke University Health System with a Durham County, North Carolina, residential address. Children with asthma were classified using previously validated electronic health record-based definitions, and SARS-CoV-2 infections were identified via positive polymerase chain reaction testing of samples collected from March 2020 to September 2021. Children with asthma were matched to those without asthma.
